SYMPTOMS
========

A stop 0x0000000A or stop 0x00000048 error may occur in the named pipe file
system (Npfs.sys).

This problem can occur when a synchronization problem causes an on thread to
complete another thread's IRP. This can cause an IRP to be completed twice. This
problem can occur on an input/output abort or cancel.
